Please read the following terms and conditions carefully before receiving any massage therapy services at Rebalance Massage. By booking an appointment and receiving massage therapy, you acknowledge and agree to the terms and conditions outlined below:
1. Scope of Practice:
1.1. The massage therapist is a trained professional who provides therapeutic massage services for relaxation, stress relief, and the promotion of general well-being.
1.2. Massage therapy is not a substitute for medical care, and the massage therapist does not diagnose or treat any medical conditions. If you have any health concerns or medical conditions, please consult with your healthcare provider before receiving massage therapy.
2. Client Responsibilities:
2.1. The client must provide accurate and complete health history information, including any relevant medical conditions, injuries, allergies, or medications.
2.2. The client must communicate openly with the massage therapist about any discomfort, pain, or preferences during the massage session.
2.3. The client must disclose any changes in their health status or medical conditions before each session.
3. Informed Consent:
3.1. The client gives their informed consent to receive massage therapy services.
3.2. The client understands that they have the right to ask questions, seek clarification, or refuse any specific massage techniques or modalities.
3.3. The client acknowledges that the massage therapist will provide draping (covering) during the session to ensure privacy and comfort.
4. Confidentiality:
4.1. The massage therapist will maintain the confidentiality of all client information, except where required by law.
4.2. The client's personal and health information will not be disclosed to any third parties without the client's written consent.
5. Cancellation and Rescheduling: to cancel or reschedule, please contact info@rebalancemassage.nl or +31 681518245.
5.1. Cancellations or rescheduling requests made at least 24 hours prior to the appointment are free of charge.
5.2. Cancellations or rescheduling requests made less than 24 hours before the appointment and no shows will incur the full charge.
5.3. Late arrivals may result in a shortened session; full service fee still applies
5.4. The massage therapist reserves the right to cancel or reschedule an appointment due to unforeseen circumstances or illness. In such cases, reasonable efforts will be made to provide an alternative appointment.
6. Payment and Fees
6.1. Full payment is required to book an appointment.
7. Professional Boundaries:
7.1. The massage therapist maintains professional boundaries at all times and expects the client to do the same.
7.2. Any inappropriate behavior or sexual advances by the client will result in the immediate termination of the session, and the client will be responsible for the full payment of the scheduled appointment.
8. Liability:
8.1. The client understands and agrees that massage therapy involves inherent risks, and the massage therapist is not liable for any injuries, accidents, or damages that may occur during or after the massage session.
8.2. The client agrees to release and hold harmless the massage therapist from any claims, demands, or liabilities arising out of or related to the massage therapy services.
9. Modifications to Terms and Conditions:
9.1. The massage therapist reserves the right to modify or update these terms and conditions at any time, without prior notice.
